Why have we made the choice not to accept insurance?

Physicians and counselors who accept insurance are required to agree to surrender
all
medical records (for covered as well as non-covered individuals) or to allow inspection of
same, at the request of the insurance company to which they belong.  We consider this
an unacceptable invasion of privacy.

We are not affiliated with any hospitals, insurance companies, or other sources who will
soon be required to surrender patient records as described in many recent news reports.
We leave the decision of whether to submit for insurance reimbursement up to our
patients who can decide for themselves how much exposure they want to have.  We do
not release information to any insurance companies and will not release to any other
entities unless a request for records for continued medical treatment is accompanied by a
signed release, or in the event of a legal subpoena or order to surrender, or in the event
that the patient poses an imminent danger to self or others.  Standard copying and/or
consultation fees apply.

We accept cash, credit cards and checks in payment for services.  Many of our patients
choose to pay in cash and not submit to their insurance for reimbursement.
Is it hard to forego receiving reimbursement?  For some.  But the additional layer of
privacy is worth it to them.

For those who choose to pursue reimbursement, we provide a receipt coded with the
information the patient will need to submit.

If you or a loved one chooses to prepay for your services, we provide an option on our
website to accomplish that goal.  Payments can be made with the credit cards mentioned
earlier.  Declined credit card transactions incur a $15.00 fee which will be added to the
patient account.

Checks that are returned as unpaid by your bank will result in a $35.00 charge.
Declined credit cards will incur an additional charge $15.00 charge per occurrence.

Why do charge for missed appointments?  Our fees are based upon the time used or the
time scheduled, whichever is greater.  The time has been reserved specifically for you and
unavailable to other patients.  Unlike other medical specialties, we can not double book
appointment slots, so when a patient late cancels or does not show for any reason, the
slot can not be filled with another patient from our waiting list.  By scheduling an
appointment, you acknowledge your responsibility to commit to being here for that
appointment time or giving
at least the minimum required notice.  In many instances,
if we are able to have contact with you, you may be offered a phone session to take
advantage of the time.  It is up to the patient whether to accept the phone session, but
doing so or not will not alter the fee for the appointment.

Outstanding delinquent accounts may be referred to a collection agency as a last resort.
Any legal fees that we must pay to secure past due balances will be added to the
account.

Regarding requests for legal involvement, responding to a subpoena and requests for
records/summary of medical records, you agree to be responsible directly for all of these
services and to prepay for them.
